The goal of using this template is to capture a clear & concise summary of what the issue is, how it impacts on, the business, what resources are initially needed (project team) & most importantly of all, get the green light to move to the next phase of the project.
Think of what departments & personnel are involved, required or impacted by this business issue. List out the project team members & key stakeholders upfront. Remember, it is much harder to get resources added later on so ask upfront.
A description of issue, including current data or facts. Clearly describe the impact of the issue on the business, highlighting in particular the cost in money & impact on customer.
Outline why the issue must be addressed, the result of accomplishing the goal, the criteria for defining success & any other associated benefits to the business.
Voice of the Customer (VOC)
A clear summary of the customer’s views, assumptions, expectations & concerns. Tools include focus groups, surveys, interviews, market research, complaint data & competitor analysis.
Cost of Poor Quality (COPQ)
Outlines how the issue is impacting on any of the key criteria to the business & the customer. Includes impact on cost, quality, delivery, satisfaction & anything else that matters.
Clear account for what process, areas & issue will be included in the project. Also clear defines what will be out of scope for project. Remember not to spread the project team too thin.
Project Plan & Sign-Off
Timing of phases for project & sign-off to officially proceed to next phase of the project.
